Before Your First Visit: Medical Records: Use the enclosed medical release for obtaining your medical records from your primary physician. Please bring us any recent lab work or reports. If we need to request your medical records, please sign the release form and return it to us ASAP. Dietary Diary: Keep a diary of everything you eat and drink for three days. Supplements and Medication: Please make a list of all prescription medications, dietary supplements, and over-the-counter drugs that you are taking. Arrival Time: Please arrive 30 minutes before your first appointment if you have not completed paperwork. This will allow time for you to fill out your paperwork prior to your visit. (If you fill out the paperwork early, then 10-15 minutes prior will suffice.) Time of Visit: Your first visit will usually last up to 2 – 2 ½ hours. Please plan for this. Fasting: Please do not eat or drink anything (except water) for at least 3½ hours prior to your first visit. It is a good idea to bring a water bottle to your visit and drink plenty of water so that you are well hydrated. Also, you may want to bring a snack with you to eat once your labwork is complete. This is necessary due to certain laboratory tests that the physician may want to perform. If you inject insulin, then call us now so we can give you specific advice. Please remember to bring your medical records, diet diary, and medication-vitamin list with you. Remember, you must drink water. As a courtesy
to our patients that have chemical sensitivities, we ask that you, and
anyone that comes with you, please not wear any perfumes, colognes or
other toiletries when you come to our office. Thank
