It is essential that you know and understand the policies of the clinic.   Please read and complete the enclosed forms before your first office visit.  If you have any questions or concerns, please feel free to contact me.  

Your appointment is with either James Biddle MD, Sharon Willingham, MD Anne Walch PA, or John Paul McNeil, PA.  Please arrive 15-30 minutes early and plan to stay at least 30 minutes afterwards.  

Bring with you the name, address, phone number and fax number of any physician you would like us to request records from, or supply the information on the enclosed records release form. 

Please have forms completed and available to give to receptionist at check -in.  We look forward to seeing you.
